You may see other people who seem to have success treating this cohort, but know that it is not easy. In general, getting patients in the door regularly does not happen overnight. It takes time and, most definitely, patience. It's important to remember this in order to not give up too soon. I don't want you to ever have any regrets about the life you've chosen for yourself.
